Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

Patients living within our practice area are welcome to register and our reception staff can confirm your eligibility from your postcode. You can check whether your address is within our catchment area on the large map behind our reception desk or see the map below. Please note that the digital map below has a small inaccuracy and the area marked here in yellow north of Chardstock is actually not in our catchment area:

If you are already registered with the Practice and move out of the practice area, you will be asked to register with a GP closer to where you live. Any patients registered with us who live outside our practice area will not be able to receive home visits, and will have to rely on NHS 111 to signpost you to an appropriate service in this instance.

There are a number of reasons why you may not be able to register with your chosen GP practice, for example you may live too far away. If this is the case simply choose another GP in your local area.

Please note that your medical records may take several weeks to arrive at the Practice. Medical treatment is available from the date of registration.
